Your referral is the greatest compliment we can receive. Our best new patients have come from our wonderful existing patients, and we created our Share A Smile rewards program with that in mind!
The program is simple: refer your friends and family to our office. You can either pass out Share A Smile rewards cards (available at the front desk), or ask your friend or family member to mention your name when they call for their appointment.

We don’t think thanking you for your referral is enough! We want your friend or family member to share in the rewards too! That’s why we’ll apply a $50 credit to their account, good towards any dental treatment.
Terms and Conditions
For each new patient, Share A Smile referral reward, the referring patient must be a current patient at Shoreline Dental Care. The new patient referred must give the name of the referrer when scheduling the initial appointment and complete the appointment (or present the Share A Smile Rewards card with the referring patients name on it). The new patient referral must complete initial consultation, initial exam and pertinent x-rays. Then, the patient who gave the referral will be notified by phone or email that they are eligible for one of the 5 Share A Smile Rewards. Once the SDC office is notified of which gift is selected, the Share A Smile Reward will be delivered to the referrer via USPS within two weeks after the appointment date, or the reward can be picked up at either of our 2 offices. Any active Shoreline patient can refer as many new patients to the office as they wish. For each new patient referred, they can select any of the 5 Share A Smile Rewards gifts.
PATIENTS WHO REFER PLEASE READ:
To ensure you receive a Share A Smile Reward for new patient referrals please note that the following must take place.
- Provide the new patient with our Share A Smile Rewards referral card filled out with your name and phone number.
- The new patient must reference either the Rewards card or your name when scheduling their first appointment.
- The new patient must present either the Rewards card or your name to our staff when checking in at their first appointment.
*Referral of a spouse and/or child does qualify for a Share A Smile Reward. Referral patient (new SDC patient) must present completed card or the referring patients name at time of first appointment for referrer (current SDC patient) to receive a Share A Smile Reward. Rewards will be given out after new patient’s first appointment. Rewards will be mailed within 2 weeks of the Rewards gift selection or can be picked up at either location. This promotion is available at both our West Haven and Milford locations. This promotion is available to active/existing patients only (existing patients are those whom have been seen by our practice within the last 18 months). The Share A Smile Rewards promotion may not be combined with any other offer or applied to prior visits. Family members of Shoreline Dental Care employees are not eligible. Must be 18 years or older to receive Rewards and/or gifts. This program is subject to change and can be discontinued at any time. Federal Law prohibits Shoreline Dental Care from offering this promotion (referral reward/gift) to anyone who is receiving services as beneficiaries under Medicare or state health programs such as Husky.
